Kentucky Department of Corrections Quarterly Food Program Information
The items in this catalog are considered Quarterly Food Program items and have been approved by the Kentucky Department of Corrections. Each facility Warden may choose to restrict items from being ordered as deemed necessary for their facility. This catalog is effective April 1 - October 31, 2024. Multiple packages can be ordered for each offender, but the combined total cannot exceed a total of $125 maximum value of product per quarter. There is a minimum order amount of $15.00 of product. Orders from family, friends and offenders will be accepted. Package privileges are subject to change without notice. It is the offender’s responsibility to ensure the items ordered are in accordance with their individual facility policy.
ORDERING INFORMATION:
Family & Friends may place orders online, by phone, fax or mail. Offenders may place orders via phone or tablet. For more information, please visit our website at www.KYpackageprogram.com. Directions for offender phone ordering can be found near the offender phones. Please use the five-digit phone item number located in the catalog when placing orders on the offender phones.
ELIGIBLE OFFENDERS: Offenders assigned to segregation status are not eligible to receive packages. If your package is refused by the offender’s facility because the offender is no longer eligible to receive an item/package or the offender no longer wants the item/package, a restocking fee equal to ten percent (10%) of the total invoice amount will be deducted from the purchaser’s refund.
PRODUCT & SUBSTITUTIONS: Union Supply Direct attempts to show and describe merchandise and all related information as accurately as possible. However, Union Supply Direct does not warrant that product descriptions or other content of our catalog, order form or website is accurate, complete, reliable, current, or error-free. In addition, manufacturers may make changes in product design, color, package size and style at any time. Union Supply Direct reserves the right to update or correct item information and to change prices and offers at any time. If a product offered by Union Supply Direct is not as described, your remedy is as follows: (1) immediately return it, unused, upon receipt while complying with your facility’s rules and regulations. (2) If an item is unavailable for any reason, we will substitute with an item of equal or greater value. We regret any of these changes and make every effort to ship the proper product that you request.
PAYMENT METHOD: Union Supply Direct accepts the following forms of payment: credit card, debit card, money order (must be domestic, printed in the U.S.), cashier’s check, personal check, facility check, auto-deduction from trust fund, layaway, PayPal and Cash via ACE Cash Express. We accept all major credit and debit cards with the following logos: Visa, MasterCard, and Discover. There is no surcharge for using your credit card to make purchases. Please be sure to provide your exact card holder name, billing address, CVV code and telephone number that your credit card issuer bank has on file for you.

LAYAWAY: The Layaway Program is for friends and family only. Family and friends (“you”) can place an order with us today to activate your layaway plan. Our layaway plan provides a monthly statement that is sent to you the first of each month throughout the duration of your layaway plan. This is a free service at no additional cost to you. Please see page 63 for details.
SALES TAX: Sales tax is required by the state of Kentucky. Candy items are considered taxable and will be taxed at a rate of 6%. All other items are not taxable. Sales tax rates are subject to change without notice.
SHIPPING: All shipped orders placed by family & friends will be charged $7.95 (Non-Refundable). Orders placed by offenders will not be charged a shipping fee. Cancelled orders will not be charged a shipping fee.
DELIVERY: Orders will be delivered to each facility on a weekly basis. Please check our website www.KYpackageprogram.com for each facilities delivery day. Orders that do not make the delivery cut off for that week will be delivered the following week. Please allow up to 2 weeks for your order to be received by your loved one.

RETURNS AND REFUNDS: Used or abused items cannot be returned, exchanged, or refunded. Please check all your items upon receipt. If any of these items are found to be missing, defective or damaged in shipping, please notify the staff inventorying your package immediately to arrange a return, exchange, or refund. Damaged or defective food and hygiene items will be refunded. If the original payment was made by credit or debit card, approved refunds will be credited back to the card used in the original purchase, including prepaid credit and debit cards. All products must come back to us directly from the facility. For security reasons, we cannot accept any product that has left the facility. All original boxes and packing material are required for any returned goods. Union Supply Direct is not responsible for refused items. Please ensure that the items you order are approved at your facility. Each facility Warden may choose to restrict items from being ordered as deemed necessary for their facility. This catalog is April 1 - October 31, 2024. Multiple packages can be ordered for each offender but cannot exceed three (3) packages per calendar quarter. There is a minimum order amount of $15.00 of product. Orders from family, friends and offenders will be accepted. Package privileges are subject to change without notice. It is the offenders responsibility to ensure the items ordered are in accordance with their individual facility policy.
